Refining radiation techniques: Focus on hippocampal sparing WBRT
نویسندگان
چکیده
Whole brain radiotherapy (WBRT) has been considered the standard treatment in patients with multiple brain metastases. In patients with a limited number of brain metastases, the use of more aggressive treatment, including stereotactic radiosurgery with or without WBRT has been proposed. Moreover, technological improvement allows clinicians to deliver simultaneously WBRT and a boost dose to brain metastases. In the last decades, WBRT has been questioned due to the presumed late decline in neurocognitive functions (NCFs); moreover, several clinical trials found relationships between hippocampal deterioration and NCFs decline after WBRT. New clinical trials are evaluating the use of hippocampal avoidance in WBRT. Nevertheless, the benefit on NCFs deficit remains unclear. Aim of the review is to analyse the role of hippocampal avoidance, the impact of technology and the current clinical trials on going for hippocampal sparing radiotherapy. Abstract

Treatment Planning and Delivery of Whole Brain Irradiation with Hippocampal Avoidance in Rats
BACKGROUND Despite the clinical benefit of whole brain radiotherapy (WBRT), patients and physicians are concerned by the long-term impact on cognitive functioning. Many studies investigating the molecular and cellular impact of WBRT have used rodent models. However, there has not been a rodent protocol comparable to the recently reported Radiation Therapy Oncology Group (RTOG) protocol for WBRT...
متن کاملHippocampal dosimetry correlates with the change in neurocognitive function after hippocampal sparing during whole brain radiotherapy: a prospective study
BACKGROUND Whole brain radiotherapy (WBRT) has been the treatment of choice for patients with brain metastases. However, change/decline of neurocognitive functions (NCFs) resulting from impaired hippocampal neurogenesis might occur after WBRT. It is reported that conformal hippocampal sparing would provide the preservation of NCFs. Our study aims to investigate the hippocampal dosimetry and to ...
متن کاملA mouse model replicating hippocampal sparing cranial irradiation in humans: A tool for identifying new strategies to limit neurocognitive decline
Cancer patients undergoing cranial irradiation are at risk of developing neurocognitive impairments. Recent evidence suggests that radiation-induced injury to the hippocampi could play an important role in this cognitive decline. As a tool for studying the mechanisms of hippocampal-dependent cognitive decline, we developed a mouse model replicating the results of the recent clinical RTOG 0933 s...
متن کاملDosimetric Evaluation of Intensity-Modulated Radiotherapy, Volumetric Modulated Arc Therapy, and Helical Tomotherapy for Hippocampal-Avoidance Whole Brain Radiotherapy
BACKGROUND Whole brain radiotherapy (WBRT) is a vital tool in radiation oncology and beyond, but it can result in adverse health effects such as neurocognitive decline. Hippocampal Avoidance WBRT (HA-WBRT) is a strategy that aims to mitigate the neuro-cognitive side effects of whole brain radiotherapy treatment by sparing the hippocampi while delivering the prescribed dose to the rest of the br...
متن کاملDosimetric analysis of the alopecia preventing effect of hippocampus sparing whole brain radiation therapy.
BACKGROUND Whole brain radiation therapy (WBRT) is widely used for the treatment of brain metastases. Cognitive decline and alopecia are recognized adverse effects of WBRT. Recently hippocampus sparing whole brain radiation therapy (HS-WBRT) has been shown to reduce the incidence of memory loss. In this study, we found that multi-field intensity modulated radiation therapy (IMRT), with strict c...
